Could be a clogged DPF that needs to be removed and cleaned/replaced - over time there is a small amount of ash that can't be removed from the filter by regen, if you have a lot of miles on it I would suspect this. Could also be a malfunctioning pressure sensor, or it could be an air leak upstream of the DPF leading to unexpected pressure differential readings. Alternatively could be a cracked or damaged DPF. If you've cleared the codes and gone through a couple regen cycles and it keeps coming back, you may need to have it inspected.
